Two great examples of accessibility fail from London:

a door to the back entrance of Great Scotland Yard, paint peeling, which contains a wheelchair icon and the sign disabled persons entrance, right above the single step to the door.

A plaque with the wheelchair icon and the phrase Welcome. Touch here for assistance. Next to restaurant which has a step up and a table with a single chair outside the step on the pavement. The plaque is, incidentally, at least 4 feet off the ground and behind a pole.

Seriously? My workplace (in Australia) had to pass an inspection to get one of those signs! Hint: we have no steps.
