We did in fact have the signs - bright yellow, and they say something like "HARD OF HEARING" in very big letters, followed by "I read lips. Face me, and speak clearly." in smaller letters, and the language services number for questions below that. There's one over the head of her bed (so that it's obvious if you're looking at her) and one on her door.
Language services also got her a visual doorbell, and a phone with amplified sound. The one thing we've had problems with is that if she hits the call button, they tend to use the intercom to respond, which she generally can't hear. (but that's been getting better, as we train rounds of staff.)
